	Media Waiver/Consent Agreement I , the undersigned, do hereby grant permission to The Central Maryland Shockers, LLC and other media representatives to photograph my child and or post my and or my child's photos videos, stories or other related items, hereinafter referred to as "Materials".I hereby release you, and any staff affiliated with the Central Maryland Shockers, LLC from all claims and demands arising out of or in connection with any use of said "Materials", including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and /or property rights. I acknowledge and agree that no sums whatsoever will be due to me as a result of the use and or exploitation of the "Materials" or any rights therein.I acknowledge that my child is under 18 years of age and lacks the legal capacity to enter into binding agreements. Accordingly, I have read this Release and consent to my child's inclusion in the "Materials" and will not contest the rights granted in this Release, and shall assist and support you in any and all legal proceeding for affirmation of this Agreement,should you choose to have a court of law affirm this Agreement.
	By submitting this form the participant assumes all risks associated with participation with Central Maryland Shockers, LLC; the Central Maryland Shockers assume no liability for injury or damages arising from participation in practice or games. Due to the strenuous nature of some activities, we encourage each participant to consult their physician regarding any medical concerns prior to participating in our program. The participant consents to emergency treatment if needed. Neither the Director nor any of the staff are responsible for children prior to or after the scheduled program. The participant agrees to abide by all practice rules and regulations set forth by the coaches. No refunds will be issued to those who are removed from practice or games for poor sportsmanship or other behavioral issues.

The Central Maryland Shockers will be participating in the HC Elite and Columbia Ravens WInter Basketball League. Please contact us for tryout schedule.

When: Starts Early December. Games will be played on Fridays and Saturdays.
Where: Howard County, Meadowbrook Athletic Complex, and/or YMCA
Practice: Twice a week.
